Where should I apply

You can apply at the consular office.

Do I need to submit documents in person

  1. Submit a Polish application for a Polish card in person-due to the need to submit a declaration of belonging to the Polish people to the consul and show the consul knowledge of the Polish language in conversation.

  2. The application for the provision of a Pole card to a minor is submitted personally by both parents of the child. A parent who does not have a Pole Card is obliged to give the consul consent to provide a Pole card to a minor.

  3. An application for the extension of the validity period of a Pole's card, for the issuance of a new card after changing personal data or for the issuance of a duplicate of a lost card can be submitted in person, out of turn, by appointment by phone.

What documents do you need to file

  1. Persons of legal age applying for a Polish Card shall submit:

  • completed application for a Polish card (below),

  • identity document with photo and copy,

  • documents confirming the Polish nationality of the applicant or his / her parents and any documents confirming kinship with the persons to whom the applicant refers, together with copies

  • or a certificate from an authorised polish or Polish organisation confirming the active involvement of the applicant in activities for the Polish language and culture or the Polish national minority,

  • ewentualnie dokument potwierdzający znajomość języka polskiego z kopią.

Dokumenty, które potwierdzają znajomość języka to świadectwo ukończenia szkoły lub studiów w Polsce, lub świadectwo ukończenia szkoły za granicą z wykładowym językiem polskim

Jeżeli nie posiadasz żadnego z wyżej wymienionych dokumentów znajomość języka oceni konsul. Rozmowa z konsulem, prowadzona w języku polskim, dotyczyć będzie m.in. polskich tradycji  i zwyczajów.

  1. Parents applying for a Polish card to minors submit:

  • completed application for a Polish card (below),

  • the minor's birth certificate with a copy,

  • identity documents of parents with copies,

  • Parents ' Polish card or one parent's Polish card

with copy / copies.

  1. Persons applying for a Polish card with changed personal data submit:

  • completed application for a Polish card (below),

  • identity document with copy,

  • a document confirming the change of personal data with a copy,

  • A Polish card with a copy.

  1. Applicants for a duplicate shall submit::

  • - completed application for a Polish card (below),

  • - I. D. with a copy.

Payment

All activities related to applying for a Polish card are free of any charges.

What is the deadline for implementation

The period for consideration of an application for a Polish card should not exceed two months. If the time limit is extended, you will be informed of the new expected time limit for the completion of the case. Please note that the date of issue of the Polish card also depends on the date of its delivery to the office.

How will you get the documents

You will receive the Polish card in person at the consular office by prior appointment by phone at: +373 22 22 38 50

How can you appeal

If you do not agree with the consul's decision, you can appeal to the Council for poles outside the country. The appeal shall be lodged by the consul, who shall give the decision within 30 days of the date of service of the decision.

On the order of the consul, you can file a complaint with the minister of foreign affairs. The complaint shall be lodged by the consul who issued the order within 7 days from the date of service.
